Billy the puppet is symbolic of Jigsaw. He delivered messages to his victims in it often via a television screen. In Saw IV, it showed that the original puppet was made by Jigsaw for his unborn child who his wife later miscarried. The original puppet was not as scary as the one in Saw III that he made solely for the purpose of using it in his bizarre killing schemes.
Even though he is one of the newer horror villains out there he is easily recognized by most people. Ask if they want to play a game and truly terrify people.
